You have to ask "per liter of what, and how much does it weigh." Parts per million is a ratio between two like quanties, weight to weight, or volume to volume, not mixed.
If it is a liter of water (or about the same specific gravity) one liter of water weighs 1 kilogram, and 1 mg/kg would be 1 ppm.
Blood, saline, etc are slightly heavier than 1 kg/L, fats and oils are slightly lighter.
