just want to know :

5 mg/ kg = 5 ppm ?

1 mg = 1/1000th of a kg, so I would think 5 mg would be 5 parts per thousand.

And 5 parts per thousand equates to 5000 ppm.

1 mg = 1/1000 of a gram, not a kilogram
and 1 kg = 1000 g, so, yeah
1 mg/kg = 1 ppm by weight.
